SIV ELIGIBILITY QUESTIONNAIRE
Instructions: Please answer these questions honestly and with as much detail as possible. The more documents you can provide, the better. Your answers, information and documents are strictly confidential. We will not share them with anyone without your permission. Only designated IRAP lawyers and law students will see them.
Identity
1. Are you an Iraqi citizen?
2. Do you have a "G" series passport?
3. Do you have access to copies of your birth certificate or your Iraqi ID card?
4. Where are you currently residing?
a. If you are not living in Iraq:
i. When did you leave?
ii. Why did you leave?
5. Are you registered with UNHCR?
6. Have you applied for a Special Immigrant Visa with the U.S. government before?
a. If yes, what is the latest information you have about your application?
7. Have you applied to the U.S. Refugee Admissions Program before?
b. If yes, what is the latest information you have about your application?
Employment
8. Have you worked directly as a translator for the U.S. military in Iraq? (for example, for particular soldiers, generals or military personnel)
a. If you have worked for different people at different times, please list every job.
b. How long did you work as a translator at each job?
c. For each job, what types of translating work did you do?
9. Have you worked directly for the U.S. government in Iraq in a different capacity after March 20, 2003?
a. If you worked for different branches at different times, please list every job.
