Dear Ms ,
Congratulations on receiving Chief of Mission (COM) approval for the Iraqi Special Immigrant Visa (SIV) program. Attached is an electronic copy of your COM approval letter. You will need to print this letter and add it to the petition file that you must now send to the United States Citizenship and Immigration Service (USCIS), which is part of the Department of Homeland Security (DHS).
To file your SIV petition you must send the following items to USCIS:
1) A completed Form I-360 (instructions:
http://www.uscis.gov/files/form/i-360instr.pdf, form:
http://www.uscis.gov/files/form/i-360.pdf);2) A copy of the passport, birth certificate or national identification card showing that the applicant is a national of Iraq, along with a certified English translation, if the document is in a foreign language;
3) A copy of the positive recommendation, confirming employment of not less than one year beginning on or after March 20, 2003, normally from the U.S. citizen who has directly supervised the Iraqi employee or supervises the company at which the worker is employed;
4) A copy of the attached “Chief of Mission Approval for SIV Status” letter.
Once you have the petition application package completed and ready to mail, please send it to one of the addresses below:

The petitions will be adjudicated at the USCIS Nebraska Service Center. Once completed, the petition decision will be forwarded to the State Department National Visa Center (NVC). You will be contacted by email from the NVC when your petition decision has been received.
